Care Coordinator — Aire Valley Community Care (sample advert)

We are looking for a Care Coordinator to join our Wakefield office. You will build and maintain the weekly rota for a team of 22 community carers, respond to changes at short notice and keep accurate records on our care management system.

You will be the first point of contact for families and for our carers in the field. Some conversations are difficult: a call-out has been missed, a family is worried, a carer is unwell an hour before a visit. We need someone who stays calm and communicates clearly.

Essential
- Experience of scheduling, rotas or coordinating people
- Confident written record keeping
- Understanding of safeguarding and when to escalate
- Comfortable using spreadsheets and a case management system

Desirable
- Experience working in a care setting
- Level 3 Diploma in Adult Care (or willingness to work towards)

Hours: 37.5 per week, Monday to Friday, with a shared on-call rota one weekend in six. Hybrid after induction: three days in the Wakefield office, two from home.
